Patients understand their rehabilitation better

First, please tell us your clinic's strengths.

Our clinic has 4 main strengths: (1) by expanding musculoskeletal rehabilitation, we now work with about 500 patients; (2) we run bone density tests regularly, and nurses also give nutrition guidance; (3) ultrasound-guided hydrorelease; (4) regenerative medicine.

So you are working with 500 patients now. Of your clinic's 4 strengths, let us hear about the expansion of musculoskeletal rehabilitation, the one about rehabilitation.

In fact, until I joined 2 years ago, the clinic had no physical therapist. Today the rehabilitation room has many beds, but when I joined, it had only electrotherapy equipment and traction, and no treatment bed.

So at first there was only 1 physical therapist? Please tell us how the number grew over 2 years.

For 1 year I worked hard as the only physical therapist. It was terribly lonely. (laughs) The loneliness is a joke, but it was very rewarding. There were few places around the clinic where people could get rehabilitation, and once we started musculoskeletal rehabilitation, I felt patients were very surprised by its effect and glad of it. We heard many positive opinions from patients. Every day I felt that patients wanted us. In the end, word of mouth spread and the number of patients grew, so 1 year later we hired 1 more physical therapist, then 2, and today we have 6 full-time and 2 part-time staff.

Your clinic has used our service since we first released it. How exactly do you use it?

We use it in 2 ways now. First, as an aid to instruction during treatment time. When patients watch an exercise on the tablet and then do it, they get a picture of the movement, so exercise instruction has become more effective than before. Second, we use it for Home ex. Until now we only told patients verbally, and they would not do their rehabilitation at home; since we started using REHASAKU, more patients do their rehabilitation at home than before.

How do patients respond?

We hear good opinions from patients, such as "it's easy to understand", "so there are handy things like this now" and "having it on paper at hand is reassuring".

What has been good about using REHASAKU?

The self-training content is simple, it is very convenient as an explanation tool, and patients understand it well. And because it is evidence-based, we can use it with confidence. Many of our patients are hard of hearing, and showing them videos makes the explanation go smoothly.

May we ask what you expect of REHASAKU from here?

Many therapists and facility staff want "more efficient exercise instruction". Being able to create in tens of seconds, within treatment time, the self-training menus we used to make after work hours makes it a very useful tool for the rehabilitation industry. In an age that demands higher productivity, I think the need for services like this will grow. I think that if its use spreads beyond medical institutions to care facilities, it will help extend healthy life expectancy. I hope it plays a part not only in therapeutic medicine but also in preventive medicine.

Your clinic opened in February this year. How has it been?

Well, the number of patients grows week by week. Though it is only 1 month since we opened, many patients come to us. When we opened, I did not expect this many. Most live within walking distance of the clinic, and patients tell us things like "we were stuck without a dedicated orthopedic clinic" and "it's good that an orthopedic clinic opened nearby".

It is impressive that so many patients have come since you opened. Does your clinic have any challenges now?

The biggest is hiring physical therapists. We currently have 1 full-time and 1 part-time. Only 1 month after opening, the physical therapists' appointment slots are already almost full. So there are patients who want rehabilitation but cannot get it. As I mentioned when talking about our patients, there is no dedicated orthopedic clinic in the area and almost no facility offering rehabilitation, so patients' need for rehabilitation is very high. For the patients in the area who cannot get rehabilitation, expanding rehabilitation and hiring physical therapists are urgent. Honestly, we are nowhere near enough physical therapists! (laughs)

So with too few physical therapists, what do you do for patients who need rehabilitation but cannot get it?

At first, for patients who could not get a rehabilitation appointment, we postponed their rehabilitation until 1 week later. As a result, some patients dropped out. So now I give patients rehabilitation instruction in the consulting room, using REHASAKU. Enter a diagnosis and a menu comes up easily, and you can also create your own combinations. In a busy outpatient clinic, it is very effective for meeting patients' need for rehabilitation. I think clinics that lack physical therapists, or have none at all, need it a great deal.

Where should 平井リハビリテーション整形外科 head from here?

We want to provide medical care rooted in the community. Around our clinic live people of every generation, from the elderly to small children. Our group includes つるかめクリニック, which provides home medical care. We want to give consistent support, from sports outpatient care for students to home medical care for the elderly.

Why exercise instruction?

To raise patient satisfaction, high treatment effectiveness and providing information are said to be important

To raise the effect of treatment, it is said to be important to do exercise including "strength training and stretching" frequently (1)(2)

So it is not only the treatment and exercise at the hospital 1 or 2 times a week: daily exercise at home (self-rehab) is also important
